Abstract: This detailed and comprehensive research text has been conducted with the aim of designing an innovative model for pathology and risk management in civil engineering projects. The case study focuses on the Shiraz-Isfahan freeway in Fars province, recognized as one of the most important infrastructure projects in Iran. The study meticulously uses a mixed qualitative-quantitative exploratory approach to understand and prioritize the internal and external risk factors impacting road construction projects. Data for this research were collected through semi-structured questionnaires. These questionnaires were carefully designed and distributed to 25 leading experts in the civil engineering and road construction industry using the Delphi method to gather their expert opinions on various topics. Data analysis was performed using advanced methods, including the Analytic Network Process (ANP) and pairwise comparisons, to achieve precise and reliable results. The research findings indicate that internal risk-creating factors in road construction projects include several issues. These factors, in order of importance, include changes in construction methods, lack of experience, design and blueprint changes, changes in management methods, project's financial credibility, unskilled workers, design errors, contractor's financial strength, rising material costs, unfamiliarity with the project area, changes in material quality, changes in organization members, and delays in material delivery. These findings clearly show how various factors can significantly impact the success or failure of a project. The research also highlights the vital role of active decision-making and appropriate budget allocation in the successful execution of projects. This highlighting provides valuable insights into potential challenges and ways to address them. Additionally, this research lays the groundwork for developing effective risk management strategies and offers practical recommendations on better managing risks in civil engineering projects. Finally, the research emphasizes the importance of a multifaceted approach in addressing challenges, including precise planning, effective communication, and active stakeholder participation. This approach can significantly contribute to improving quality and reducing risks in civil engineering projects.
